1. Permit Receiver / Permit Holder
Responsibilities:
Receives and understands the Permit to Work (PTW) before starting the job.
Ensures all safety procedures and conditions are met.
Communicates hazards and controls to their team.
Stops work if conditions change or become unsafe.
2. Permit Issuer / Permit Authorizer
Responsibilities:
Reviews job scope, hazards, and risk assessments.
Verifies that isolations and safety measures are in place.
Issues the permit to a qualified receiver.
Often a supervisor, safety officer, or operations authority.
3. Area Authority / Site Supervisor
Responsibilities:
Controls activities within a specific work area.
Coordinates multiple permits to avoid conflicts.
Ensures safe integration of simultaneous operations (SIMOPS).
4. Safety Officer / HSE Officer
Responsibilities:
Audits and monitors compliance with PTW procedures.
Provides safety oversight for high-risk work (hot work, confined space, etc.).
5. Operations Supervisor / Control Room Operator
Responsibilities:
May be involved in isolations, lockout-tagout (LOTO), or providing final go-ahead for work.
